12th Grade Required Vaccines
The summer is an exciting and busy time for families as they look forward to their student senior year! As you are scheduling medical/dental appointments during the summer to avoid the back-to-school rush, we want you to be aware of the required vaccines for 12th grade students:
Booster dose of meningococcal vaccine
Hepatitis A vaccine (2 doses)
(This has been added as a required vaccine starting with the 2018-19 school year)
Immunizations may be obtained from your child's medical provider or from the Vanderburgh County Health Department's Immunization Clinic. You may phone 812-435-5997 to schedule an appointment with the Health Department. Please send updated shot records with your student on the first day of school to give to the school nurse of fax the records to 812-435-8875.

Back to School Nurse Information
All freshmen and new students entering EVSC are required to have a school physical, and all students need up-to-date immunization records. It is very important that students with allergies, asthma, diabetes, and seizures have a new and current action plan on file in the nurse's office at the start of each school year. If you doctor needs a copy of the action plan, you may pick one up in the nurse's office. If a student requires any type of medication, all of these need to be in the nurse's office at the start of each school year in the original bottle along with written permission of a parent or guardian stating what the medication is for, the correct dose, and time it should be administered.
